There is an annual Open Enrollment period for all Affordable Care Act qualified
plans. Dates may vary. To begin coverage outside of Open Enrollment, you must have a
such as a birth, marriage, divorce or other life event. You must apply for coverage and send proof of
the qualifying event within 60 days of the event. We will not approve your application until we receive that proof.
All Affordable Care Act qualified plans include mandatory Essential Health Benefits. Plans
not sold on the Health Insurance Marketplace also include pediatric dental coverage. If you
already have a stand-alone pediatric dental plan, you may purchase a plan without it by
providing a copy of the certificate of creditable coverage. Medical coverage for eligible
dependents expires on the last day of the month of their 26th birthday. Coverage for pediatric
dental and vision expires on the last day of the month of the dependent’s 19th birthday.
Rates are based on where you and your covered dependents live in Arkansas, tobacco use, benefit
plan design, network chosen, pediatric dental coverage and any optional Riders. If you do not want
to purchase a Rider, you must tell us by checking "No." (Riders no longer available starting 1/1/2015.)
